PROCEDURE DETAILS
1. Prepare the Achievement Review Form
What to Do:
Prepare the Achievement Review Form.
How to Do It:
Complete the Achievement Review Form by filling in the following information:
Staff name
Job title
Department
Starting date
Date of last evaluation
Assigned job duties and responsibilities
Why:
All achievement reviews must be completed by the required deadline. All information discussed during the evaluation interview must be treated with strict confidentiality.
2. Conduct the Staff Interview
What to Do:
Interview the staff member for performance evaluation.
How to Do It:
Meet with the employee and review performance based on assigned job duties.
Evaluate and discuss work performance, strengths, and areas requiring improvement.
Explain the results clearly and professionally.
Select the appropriate performance rating level on the form.
If improvement is needed, set a target date and clear expectations.
Encourage the employee to provide comments and record them in the appropriate section.
Request the employee’s signature to confirm the discussion has taken place.
Why:
The evaluation process must be conducted in a friendly and approachable manner, while remaining fair, tactful, and professional. Clear judgment and complete documentation are required, following the standard training and evaluation procedure.
3. Submit the Completed Form
What to Do:
Forward the completed Achievement Review Form to the F&B Manager.
How to Do It:
Submit the completed form to the F&B Manager for review and signature.
Send the original signed document to the Personnel Office.
Send a copy to the Training Department.
Keep one copy with the Outlet Manager for reference.
Provide one copy to the staff member.
Why:
All documents must be submitted on time and must be complete, clear, and legible to ensure proper record keeping and follow-up.
